Background In the transition from the sixth to the seventh edition of the American Joint Committee on Cancer (AJCC) melanoma staging system, mitotic activity was incorporated, while Clark level of invasion was abandoned. Objectives To investigate the effect of this change on the pathological tumour (pT) 1 substaging of primary cutaneous melanomas and the possible clinical implications. Methods Patients with pT1 melanomas, diagnosed in the period January 2003 to March 2011, were selected from a population-based cohort study on cutaneous melanoma in the eastern part of the Netherlands. The pT1 melanomas were systematically reviewed by an expert pathologist and classified according to both the sixth and the seventh editions of the AJCC staging system. The shift of melanomas between pT1 substages, classified according to the two staging systems, was determined. Results In total, 260 pT1 melanomas were included. Overall 28% (57/207) of all pT1a melanomas shifted to pT1b when classified according to the new seventh staging classification, because of the presence of mitoses. Some 32% (17/53) of all pT1b melanomas shifted to pT1a. The percentage of pT1b melanomas relative to all pT1 melanomas increased from 20% to 36%. Conclusions The addition of mitotic activity to the pathological staging system, according to the seventh edition of the AJCC staging system, resulted in a considerable change in the classification of thin cutaneous melanomas. This shift has clear clinical implications, as it is advised in the Dutch guideline that patients with pT1b melanoma should be offered a sentinel lymph node biopsy.

AirTree is preparing for land-based testing at the NIOZ/IMARES test facility. As the Ballast Water Management System (BWMS) developed by AirTree uses ozone as active substance, IMO Guideline G9 applies and Basic and Final Approval are also required. In preparing the Basic Approval dossier, it appeared that too little is known about the development of disinfection by-products (DBP) during the treatment process.
Seed governance. From seed aid to seed system security in fragile areas

Intergovernmental agencies and development organizations, including Cordaid, consider interventions directed at seed security of utmost importance to support smallholders recovering from conflict situations and disasters, and to contribute to revitalisation of local agricultural production and food security. There is, however, considerable debate about the most appropriate type and strategic level of intervention to enhance smallholders’ seed security in conflict and post-conflict areas. Given the co-existence of different types of interventions and agencies directed at providing seed security, the governance of seed security has become very relevant, questioning what collaborative arrangements between government, business and civil society can help to effectively address seed insecurity. Donors and development practitioners often prefer certified or improved seed to seed from the informal sector. However, farmers’ evaluation criteria can differ from criteria developed by breeders or those setting seed certification standards, and the agro-ecological conditions under which varieties are selected may differ from those on-farm, thus affecting crop performance.

Report on developing guidelines and procedures for the pesticide registration in Ethiopia in a scientifically underpinned way and as specific as possible for Ethiopian conditions. It covers the aspects of efficacy assessment, human health risk assessment as well as environmental risk assessment. It aims to develop the capacity at the APHRD to apply these guidelines and procedures. The Work package will result in an evaluation manual plus software tool for the Animal and Plant Health Regulatory Directorate (APHRD) of the Ministry of Agriculture of Ethiopia. The user-friendly software tool operationalises the guidelines and procedures for human health risk assessment and environmental risk assessment; all basic pesticide data can be entered in it and these will be used for the risk estimates.

A serious complication seen in critically ill patients is the solidification of enteral nutrition causing gastrointestinal obstruction. It has been suggested that enteral nutrition enriched with insoluble fibers may increase the risk of this complication. Therefore, we investigate the effect of soluble and insoluble dietary fibers on the coagulation of a casein-based enteral nutrition in an artificial gastric digestion model. A 100% casein-based enteral nutrition was enriched with increasing concentrations of soluble fibers (acacia fiber, oligofructose and inulin) and insoluble fibers (soy polysaccharide, resistant starch and alpha cellulose). After digestion in an artificial gastric model, the chyme was poured over sequentially placed sieves, separating the coagulate into size fractions of larger than 2 mm, between 1 and 2 mm, and between 0.25 and 1 mm. Of these fractions we measured wet weight, dry weight and protein content. A significant effect on the fraction larger than 2 mm was considered to be clinically relevant. Addition of high concentrations soy polysaccharide and resistant starch to a casein-based enteral nutrition, did not alter the wet weight, whereas dry weight and protein content of the coagulate was significantly reduced. When high concentrations of soy polysaccharide and resistant starch are added to a 100% casein-based enteral nutrition, the coagulate consist of more water and less proteins, which may lead to an increased protein digestion and absorption in a clinical setting. The suggestion that insoluble fibers increase the risk of gastrointestinal obstruction in critically ill patients is not supported by these data.

Background/Objectives: The habitual consumption of a specific type of alcoholic beverage may be related to the overall dietary pattern. The objective of this cross-sectional study was to investigate associations between alcoholic beverage preference and dietary intake in The Netherlands. Subjects/Methods: A total of 2100 men and women from the Dutch National Food Consumption Survey 2007–2010 were studied. A general questionnaire assessed alcoholic beverage preference and two non-consecutive 24-h dietary recalls assessed overall diet. Mean nutrient and food group intakes, and adherence to the 2006 Dutch dietary guidelines across categories of alcoholic beverage preference were compared and adjusted for age, sex, body mass index (BMI), education, smoking, physical activity, energy intake and frequency and absolute alcohol consumption. Results: Largest differences in dietary habits were detected between persons who preferred wine and those who preferred beer. Persons with a beer preference had a higher absolute intake of meat, soft drinks, margarine and snacks. In contrast, persons with a wine preference had a higher absolute consumption of healthy foods. However, after multiple adjustments, wine consumers still consumed less energy and more vegetables and fruit juices compared with beer consumers. Adherence to the Dutch dietary guidelines did not differ between preference categories after multiple adjustments. Conclusions: In this cross-sectional analysis in a representative sample of the Dutch population, a beer preference was associated with less healthy dietary behaviour, especially compared with wine preference. However, these differences were largely explained by other socio-demographic and lifestyle factors. These results suggest that alcoholic beverage preference may not be independently related to diet.
