This publication describes his work as a PhD student in the Host-Microbe Interactomics Chair group at Wageningen University within the Gastrointestinal Health theme. It has been completed under the supervision of Prof. Dr Jerry M Wells, Dr Jan Dekker and the TIFN project leader, Prof. Dr Paul de Vos.

Mucus serves as a protective layer between the intestinal content and the intestinal wall. It facilitates the passage of the luminal content through the intestine, reducing the risk of mechanical damage to the intestinal epithelium. The overarching goal of this thesis was to investigate the role of mucus in the maintenance of the intestinal immune barrier and the effects of ageing and gender differences on mucus production and the gut barrier.

We found by using a mouse model that decreased mucus production leads to changes in microbiota and mucosal stress responses, without the appearance of pathology, demonstrating the importance of mucus in intestinal homeostasis. The mucus barrier was shown to deteriorate during aging but this could be prevented with specific probiotics. Furthermore gender-specific differences in the effects of ageing on the mucosal barrier were found. Increased knowledge on these mechanisms might contribute significantly to disease prevention and treatment, for instance by optimizing gender-specific dietary and pharmacological requirements.

In many places in the world, increasingly less chemical crop protection agents are available for use in mushroom cultivation. As a consequence, mushroom cultivation will loose the ability to use crop protection agents. As a consequence, good hygiene management, early detection and monitoring of pathogens (diagnostics), alternative crop protection agents and disease resistant mushroom varieties have to play an important role. The number of researchers in several mushroom producing countries is decreasing already for a number of years. For an effective use of research funds, international collaboration on topics of mutual interest is important. At the workshop of the Global Mushroom Disease Diagnostic Initiative, held in 2008 just before the ISMS conference in Cape Town, it was decided to try and build a collection of reference pathogen strains for the white button mushroom diseases present world wide. This collection will play a vital role in the development of diagnostic methods for mushroom diseases. This project was funded in part by the ISMS and in part by the Dutch Horticultural Board.

In recent years, several other vectors have become world news: the sheep tick was discovered to be the vector of Lyme disease in 1980; the mosquito Culex pipiens began transmitting West Nile virus in North America in 1999; in August 2006, bluetongue virus was discovered in Belgium and the Netherlands, transmitted by biting midges and in 2007, Asian tiger mosquitoes began transmitting Chikungunya virus in northern Italy. Buruliulcer (BU), the common terminology for the disease caused by Mycobacteriumulcerans (MU) infection manifests as disfiguring skin ulceration which is difficult to treat. In its advanced stage the disease does not respond to drugs and requires surgery, often limb amputation. It sometimes results in death. It is most widespread inWest Africa.

InGhana, the first BU case was reported in 1971 and between 1991 and 1997 more than 2000 cases were reported. Approximately 6000 cases were recorded in a national survey in 1999. Of the 110 districts inGhana, at least 90 of them were found to have BU cases of whichAmansieWest District (the study area) had the highest rate with a prevalence of 150.8 per 100,000. The district is made up of about 310 settlements with a total population of about 108,726. High incidence of BU occurs in settlements in close proximity to theOffinRiver.

Infection is acquired through MU in the natural environment. Knowledge gaps about the exact mode of transmission and factors that pre-dispose to infection motivate this study. It employs a spatial approach to the relation between BU and postulated risk factors in part of theAmansieWest District of Ghana.

Arsenic is implicated in several types of skin diseases including skin cancers. This may be due to its immunosuppressive affects, which enhance susceptibility to infection. This study takes as risk factors different pathways of potential exposure to enhanced levels of arsenic in the environment.

Arsenic concentrations in stream sediment were used to infer relative arsenic levels in surface waters and floodplain soils. The incidence of BU per settlement was then related to the arsenic levels of these domains by proximity analysis. The results showed that mean BU prevalence in settlements along arsenic-enriched drainages and within arsenic-enriched farmlands is greater than elsewhere. However, mean BU prevalence is greater along arsenic-enriched drainages (0.7%) than within arsenic-enriched farmlands (0.6%).

Arsenic concentrations in surface (stream) waters and ground (wells) waters were determined for abstraction points used by inhabitants of settlements of the study area. These arsenic data and BU prevalence per settlement were employed in exposure-responsemodellingusing linear regression. The results showed that BU prevalence has a significant positive relationship with arsenic levels in surface water ( R 2 = 0.82, p < 0.05) but not with arsenic levels in groundwater ( R 2 = 0.10, p = 0.60).

Arsenic liberation into the environment by oxidation ofarsenopyriteduringartisanalgold mining rendersminesitesa postulated risk factor for BU. Cases increased since the promulgation in 1989 of the Small Scale Mining Law, which legalized and increased such mining. The results of proximity analysis showed that most settlements relatively near toartisanalminesitesexhibited disproportionately higher BU incidence ( R 2 = 0.48, p < 0.01) than elsewhere in the study area ( R 2 = 0.00, p = 0.90).

An integrated spatial-statistical model showed that settlement elevation, inferred arsenic floodplain soils, and distance tominesiteswere significantly correlated with BU. Surface water has a positive association with disease but not at statistically significant level. The results suggest that chronic ingestion of elevated concentrations of arsenic in drinking water and of arsenic taken up byfoodcropsproduced in arsenic-enriched soils adversely impact human health, perhaps by disruption of immune mechanisms, compromisingdefenceagainst opportunistic infections such as MU.

Nutrition Communication in Dutch General practice: integration of the patients' perspective and the family doctors' perspective

A sound nutrition advice is important for patients with coronary heart disease, diabetes and obesity. The family doctor has high potential to promote nutrition to their patients through its broad reach, including the hard-to-reach low socio-economic class. Unfortunately, in daily practice of most family doctors, this nutrition communication is not optimal. Therefore, a combination of qualitative and quantitative studies among patients and family doctors was performed in order to explore and assess their communicative characteristics regarding nutrition communication.

In our literature review, we searched for studies about the occurrence, the patients' perspective and the family doctors' perspective regarding nutrition communication in general practice. It can be concluded that results about the occurrence of nutrition communication in general practice differed widely. Moreover, patients' perceptions regarding nutrition communication through family doctors appeared to be positive. Family doctors' perceptions were positive too, but they also experienced several barriers, such as lack of time and patient non-compliance.

To explore and assess the patients' perspective, we used focus groups, in-depth interviews and face-to-face interview-assisted questionnaires. A discrepancy between perceived relevance and information needs regarding food topics was discovered. Patients' perceptions regarding nutrition communication through family doctors were positive. Whereas patients had difficulties with estimating their nutrition behaviour, nutrition awareness is important as a first step in behavioural change. With our hypothetical model, we were able to unravel the concept of nutrition awareness. Individual variables were more prominent than environmental variables. In addition, gender and age added to our model. Total explained variance appeared to be 54%.

In order to explore and assess the family doctors' perspective, we used focus groups and questionnaires. These studies showed that family doctors' were positive about nutrition communication towards their patients. However, they did not always feel capable to communicate about overweight. Moreover, family doctors used a combination of nutrition communication styles, namely informational, reference, motivational, confrontational and holistic nutrition communication styles. Our hypothetical model was tested for each nutrition communication style and showed explained variances up to 57%. Individual variables were the best predictors and socio-demographic variables did not add to the models. It is recommended that family doctors become convinced that patients prefer them for nutrition communication. Raising nutrition awareness among patients is necessary. Finally, we advise that family doctors realise that they can apply different nutrition communication styles.
